Patient Management Program (PMP)
At our specialty pharmacy we believe each patient's medication therapy is optimized through partnering with all members of the healthcare team, which includes the patient, pharmacist, and prescriber, to make the best informed decisions. Ocean Breeze offers a Patient Management Program (PMP) for those who receive specialty medications from our facility. The PMP provides a variety of services to help patients to understand, manage, comply, and plan their medication therapy.
Most importantly, an individualized treatment of care and personalized case review with follow up from a Clinical Pharmacist every three to six months will take place and requires active patient participation.

The PMP includes services that are free of charge:
- Medication compliance/adherence
- Medication therapy review
- Patient education on appropriate medication use, administration, and side effects
- Coordination of care and services with the prescriber, clinical pharmacist, and manufacturer
- 24/7 access to a clinical pharmacist
- Ongoing reassessment of individualized plan of care and regular follow up calls
Every patient receiving a specialty medication will automatically be enrolled into the PMP program unless you opt out.
If you opt out of the program you will no longer receive routine follow up calls or emails from our clinical staff or be sent any patient satisfaction surveys.
All patients will still have access to routine pharmacy services such as refill reminders, on-call pharmacist 24/7, and patient education tools even if you choose to opt out of the Patient Management Program.
